2. Health Information and Authorized Pickup

Medication assistance or administration must be separately arranged with the director before attendance. Listing medication here does not establish an approved medication plan.
Authorized Pickup Adults – Include parents who may collect the child.
Adventure Academy LLC will verify pickup identity and authorization. Communicate changes directly to the director. Provide applicable pickup restrictions and court documents privately before class; Adventure Academy LLC staff will follow applicable legal requirements.
3. Activities, Risks, and Emergency Care
Authority: The signer must have authority to consent to participation and medical care, and authority under applicable Florida law to execute the minor-child release. Any limitations on that authority must be disclosed before signing.
Program Activities
Hiking; fire making; knife handling; self-defense and combatives; sewing and stitching; beekeeping; foraging; farming; surfing; swimming; sun and ultraviolet exposure; exposure to marine life and surf conditions; exploring natural environments; physical activities; exposure to wildlife; and other activities in nature within the forest and on the beach.
Acknowledgment of Risks
I understand that outdoor activities can involve uneven ground, falls, sharp tools, fire and burns, bites and stings, allergic reactions, plants and wild foods, heat and sun exposure, changing weather, currents, waves, and underwater hazards. Possible consequences include cuts, fractures, illness, serious injury, drowning, permanent disability, or death. Emergency help may take time to arrive.
Participation and Safety
I voluntarily permit my child to participate in the confirmed program activities. I will provide accurate safety information and discuss the rules with my child. My child must follow Adventure Academy LLC staff directions, use required protective equipment, and promptly report injuries or concerns. I understand these precautions do not eliminate every inherent risk.
Medical Treatment Authorization
If my child becomes ill or injured, I authorize Adventure Academy LLC staff to provide first aid within their training and to contact emergency services. I authorize emergency evaluation, transport, and treatment by qualified medical professionals as permitted by law. Adventure Academy LLC staff will make reasonable efforts to reach me or the emergency contacts, but necessary emergency care should not be delayed while attempting contact. This authorization does not replace a separately agreed medication plan.
Why This Notice Is Included
The following notice is included under Florida Statute section 744.301(3), which addresses parental waivers for the inherent risks of activities offered by commercial providers. It explains that some risks remain even when reasonable care is used and that signing this agreement releases certain rights to seek compensation for harm arising from those inherent risks. Please read the notice and the agreement carefully, and ask any questions before signing.
Notice to the Minor Child’s Natural Guardian
Read this form completely and carefully. You are agreeing to let your minor child engage in a potentially dangerous activity. You are agreeing that, even if Adventure Academy LLC uses reasonable care in providing this activity, there is a chance your child may be seriously injured or killed by participating in this activity because there are certain dangers inherent in the activity which cannot be avoided or eliminated.
By signing this form you are giving up your child’s right and your right to recover from Adventure Academy LLC in a lawsuit for any personal injury, including death, to your child or any property damage that results from the risks that are a natural part of the activity. You have the right to refuse to sign this form, and Adventure Academy LLC has the right to refuse to let your child participate if you do not sign this form.
4. Limited Release, Refunds, and Acknowledgment
Limited Release
In consideration of participation in the Fall 2026 program, I, as the child’s natural guardian with authority under Florida law, release Adventure Academy LLC, on behalf of my child and myself, from claims for personal injury, including death, and property damage resulting from inherent risks of the activities, only to the extent permitted by section 744.301(3), Florida Statutes. This release does not waive claims beyond the scope permitted by that law, including claims arising from the provider’s failure to exercise reasonable care. No provision waives rights that cannot lawfully be waived.
No Refunds – Withdrawals and Missed Classes
Enrollment payments are non-refundable for voluntary withdrawal, absences, tardiness, or missed classes, except as required by law. Dismissal is addressed below.
Dismissal
Adventure Academy LLC may immediately remove a student from an activity for safety reasons. Serious or repeated unsafe behavior, bullying, or refusal to follow safety instructions may result in dismissal after communication with the parent; serious incidents may require immediate dismissal. No refund is provided for dismissal for misconduct, except as required by law.
Acknowledgment
I have read the complete agreement, including the Notice to the Minor Child’s Natural Guardian. I have had the opportunity to ask questions and understand the limited rights being released. I confirm that the information provided is accurate and that I have authority to sign. I understand that participation is voluntary.
